Enrollment Forms
Enrollment forms for the 2012-2013 school year are available for download through our website. Each year new forms need to be filled out by parents or guardians to give HOPE the most current information on your child/children who will be attending the preschool.
Follow these easy steps:
- Download enrollment forms from the link below
- Fill in the forms by typing your responses in the highlighted fields. Since your responses won’t automatically “wrap,” manually place your cursor on each line to ensure your entire response is printed.
- Print the forms
- Sign and date the forms
- Return forms to HOPE by August 1, 2012 either in person or by mail to: 43454 Crossroads Drive, Ashburn, VA 20147
Every family should return Forms 1 – 4 AND the Virginia Health Form (Form 5) for each child attending HOPE. If your child requires an Epi-pen, then you must also fill out the Written Medication Consent Form which must be signed by you and your physician. Please note this form is only valid for six months.
